What We Do: A Life Care Plan

The overall goal of the Life Care Plan is to help seniors and their families to navigate the senior care system more easily and quickly, and to enhance and protect the quality of life of the senior.

The Life Care Plan is a three-step process. The first phase is crucial for this plan.

  • Step 1: Assessment. This step involves a comprehensive assessment of the older person’s health and physical abilities, the current home and social environment, financial resources, existing legal documentation, as well as the preferences of the elder client and concerns of family members.
  • Step 2: Recommendations. After the initial assessment, we will evaluate and analyze the information and concerns you have shared with us. With all this in mind, we will then put together a personalized package of recommendations, including residential and community care settings in your area, including advice on how to choose and pay for the best facility or care providers. In addition, our recommendations will address preparing for the future, through protection of assets and execution of legal tools to implement and safeguard the elder client’s wishes.
  • Step 3: Follow-up. The third phase involves a follow-up discussion of the care decisions that have been made and other issues that have arisen that may need further attention.
